Joseph Cohen is the principal of Error Analysis, Inc. His work focuses on safety applications, incident investigation and liability, and the influence of factors that shape human behavior and  human performance. Joseph has partnered with clients in 13 U.S. states, the Netherlands, United Kingdom, Ireland, and Singapore during his 20-plus year career. Attorneys representing plaintiffs and defendants have retained Joseph in civil litigation as an expert consultant and/or witnesses. Joseph co-edited the e-book, The Human Factors and Ergonomics Society’s Guide to Forensic Human Factors (1987-2021). Joseph served as the President of the Board of Certification in Professional Ergonomics in 2014 and 2015. His formal education includes a BA in psychology from the University of California, Riverside, a MA in psychology (applied, experimental and human factors option) from California State University, Northridge, and MS and PhD degrees in industrial/organizational psychology from Alliant International University, San Diego. He is a Certified Human Factors Professional (CHFP).

Cindy’s research and consulting activities include but are not limited to pedestrian safety, ladders, and warnings and risk communications. She has authored or co-authored several peer-reviewed writings on pedestrian falls. Cindy began her career as a software quality engineer with General Dynamics in San Diego, CA, later shifting her emphasis to human factors engineering prior to her current role as Senior Consultant with EAI. She holds a B.S. in Industrial and Systems Engineering from The Ohio State University with an emphasis in Human Factors Engineering and an M.S. in Systems Management from the USC. She is a member of the Human Factors and Ergonomics Society. She is a Certified Professional Ergonomist (CPE) and a Certified XL Tribometerist (CXLT). 

H. Harvey Cohen

Founder (retired)

Harvey Cohen founded Error Analysis, Inc.  His work in human factors and safety significantly advanced its application in claims and litigation, most notably in the areas of occupational safety and personal injury.

Following an early experience at the Langley Research Center of the National Aeronautics Space Administration (NASA) and the National Institute for Occupational Safety and Health (NIOSH) in the 1970s, Dr. Cohen co-led Safety Sciences (San Diego, CA) on research funded under contract by various public and private sources. He later formed Cohen and Associates (which later incorporated as Error Analysis, Inc.) to further its litigation practice. He served as a subject matter expert and/or consultant to legal practitioners in nearly all 50 states in America, four U.S. territories (e.g., Guam), and three Canadian provinces during his 50-year career.
